We have over a decade of experience treating home-bound patients and working with home health agencies.

We coordinate the care between the caregiver, home health, DME, primary care physicians, and pharmacy to facilitate all of the components needed to safely and effectively care for the home wounded patient.

We provide in-person services as well as services via telemedicine.

Using our state-of-the-art EHR, wound care orders and progress notes are sent in real-time at the time of service to the Home health agency for quick processing as well as a detailed report of patients progress to the primary care physician.
